In collaboration with Center of Hope Youth Advocacy, LLC., as a subcontractor, STARS Mentoring Services, LLC provides Targeted Case Mangement Services in the following counties: Charleston, Berkeley, Dorchester, Colleton, Williamsburg, Horry and Marion.

What to expect with targeted case management services
STARS Case Managers work to provide a coordinated system of accountability for ensuring all necessary services are accessed, coordinated and delivered in a strength-based, individualized manner.
Some things your case manager may assist you with:
- Assessing needs and completing an assessment
- Developing an individual plan
- Coordination of services
- Ongoing monitoring and follow-up
- Working with you to develop goals
- Link you with medical, social, educational and other providers
- Ensuring the services you receive are beneficial to you
The Components of Targeted Case Management are:
- Assessment

- Referral and Linkage
